The questionnaire asks you why you want to work at the company and how it would help you with your goals, why you're interested in entertainment, what motivates you, etc.
I lived in the midwest and the office is obviously in California - that made things difficult. Luckily the person hiring was able to meet with me later in the day and we sat down for maybe 20 minutes. He asked basic questions about my resume and then offered it to me in the room. It was really simple - easy to get internships there.
