After applying online, I received an email within 1 day from the Talent Acquisition group asking a response to the following questions:
Please e-mail me back the following information at your earliest convenience and attach a resume in Word format to your e-mail response:
1. What is motivating you to consider new opportunities at this time?
2. What is your current salary including any incentive compensation? What are your salary expectations? Please be specific. We have positions at many levels and commensurate with an individual’s experience and skill sets.
3. How much notice do you require to conduct a phone screen interview?
4. Are you within driving distance to Boston, MA or would you require a relocation package?
5. Do you have at least 5 years of professional experience in a technology support and or software engineering role
6. How many years experiences in a QA leadership or management role
7. How many years of direct working experience in working in an Agile environment
8. Do you have direct working experience in testing all phases of the SDLC
9. Have you developed and maintained automation scripts using tools like Cucumber, FitNesse, TestNG, and Selenium
10. Do you have a bachelor’s degree in Computer Science or Information Services Management
11. Are you eligible to work in the United States for any employer? If not, what type of immigration Visa would you need in order to work at our company?

Within a day of answering these questions, the company setup a phone screen.
The phone screen with the hiring manager was short and they asked 5 questions (see below). The questions were process only and not technical. At the end of the phone screen I was led to believe that there would be a next step but the company went silent. I have tried communicating through email but unfortunately never received a response.
