I applied online. The process took 5 days. I interviewed at Interac (Shibukawa, Gumma) in Feb 2012
Interview
Part of the application package was a video of myself demonstration parts of a lesson (demonstrating an activity). My application was accepted, and an interview was conducted over Skype since I did not live near Tokyo at the time. I was asked a series of question about teaching English; how I would handle working with children; working in Japan and my Japanese language skills; and things I would be comfortable doing, i.e., teaching solo or with a JTE.

Accommodating hiring officer with concise information about the workload and workplace. They also follow up on the application respectfully and gives instructions the best way possible. I am very satisfied with their overall approach.

Interview
Straightforward and pretty informal to say the least. Questions were basic but felt more like a conversation with a friend. I guess they want to make you as comfortable as possible during the interview process. Interview is not so bad even for those who are shy or introverted. They really want to hire you, and as long as you can answer simple questions, you should be solid.
