I applied online. The process took 2 weeks. I interviewed at Missionary Training Center (Provo, UT) in June 2014
Interview
First round I went in and they had me teach something from PMG fairly briefly. I had 7 minutes to teach something and have it be relevant to them personally. The second round is also teaching from PMG, but a little more in depth. If things go well, then they ask you a lot of scheduling questions, etc.
