I applied online. The process took 1 week. I interviewed at Ovation Networks (Cedar Rapids, IA) in Mar 2014
Interview
I applied for the position and was contacted the next morning. Apparently the open position was for someone leaving on Friday (it was Tuesday), and they wanted to interview me immediately.
The interview started with the project manager, who stated up-front that he was looking for a leader to help budget and plan their engineering team (which would be me and one other guy). Then the two engineers (one hardware, one software...who was leaving) came in and asked me detailed questions about my experience.
After that, I was shown the office and the project (Wi-Fi enabled thermostats), and talked a bit more with the project manager. Interestingly enough, I was not introduced to the owner (there's only 30 people that work there).
All the software they used was open-source, and there were very few development and test tools. I was told the budget was extremely tight.
By the end of the interview, it was obvious they needed a System Architect as well as a Software Engineer, but I didn't want to hurt anyone's ego (I was told that was an issue with a former employee).

Question 1
They asked me take a FizzBuzz test. It's common. Google it.
