Description CarePartners is one of the largest, most reputable, and well-established accredited home care and community service providers in Ontario. We provide nursing, personal support, therapy and rehabilitation support services for patients of all ages. Each year, we average almost 4.3 million visits for approximately 72,000 patients in homes, schools, clinics, workplaces and retirement homes.
Throughout our long history in home health care, CarePartners has never been complacent. We constantly innovate to meet patient needs with new care models, new testing protocols, enhanced in-house training and virtual care technology. Over the years, we’ve designed innovative programs in collaboration with Ontario’s Home and Community Care Support Services (HCCSS), such as Virtual Care for nursing and rehabilitation, award-winning eHomeCare for palliative and end-of-life patients and their caregivers, a Stroke Pathway model, a home support worker program, and an Alternate Level of Care Transition Care model for patients requiring care after hospital discharge. More recently, we launched Ontario’s first and only modular community nursing clinic. And in early 2023, we were the first community nursing organization to implement ultrasound guidance to start peripheral IVs in the home and clinic setting in Ontario.
CarePartners advocates tirelessly for improved home health care services and a stronger, more accessible and integrated health care system for Ontarians by working closely with several Ontario Health Teams, 14 Home and Community Care Support Services, Home Care Ontario, the Canadian Home Care Association, other service providers, Public Health, Ministry of Health, Ministry of Long-Term Care and hospital boards. Through our charitable arm, Community Nursing Services (CNS), CarePartners and its employees devote their time and money to regions all over the globe who lack adequate access to health care.
We continuously strive to make health care accessible for all by actively partnering with schools, hospitals, retirement homes and communities, post-secondary institutions for nursing and other clinical programs, pharmacies, technology providers, Indigenous groups and immigration services.
