New hires are fresh out of college and do not have enough experience which really affects the quality of work that you are then responsible for. There is no training/mentoring in place at any level so everything is trial by fire. They lay off/dismiss people quite often to cut cost consequently no one is willing to own up to mistakes and moral is extremely low. Lower Mgmt is so concerned with costs that they have no idea what is going on in their own departments.
