- Process not well validated, a lot of human intervention needed thus operator mistakes is bound to happen - Too many management positions, the amount of managers is as much as operators at some departments, making promotions sometimes only by technicality not responsibility - Operating at almost full capacity yet very little manpower. Operators are expected to multitask but if anything goes wrong operators gets blamed because no one is checking others' work
