The tech is literally built by generations of inexperienced engineers inheriting tech debt, rewriting it poorly, and repeating the cycle. Good engineers are rare and do not stay for long.
Ultimately it boils down to poor management. There are little consequences or rewards to under or over performing. Good engineering is not noticed let alone rewarded. Promotions are based more on politics than impact.
