The HR and management teams do not see fit to address matters in a timely fashion and sometimes not at all. They prefer to sweep issues under the rug and ignore problems unless repeatedly reminded of them. If you are not a butt kisser and "pretending" to be doing your job, when actually screwing around, you are seen as a trouble maker for turning the bad people in. If you bring issues to their attention, such as a sexual harrassment claim, nothing is done about it until they are pushed, but then you are seen as a whiner and trouble maker. HR seems to talk to others about issues that should not be discussed outside of their department.
