Nepotism. People that stick around are elevated to positions that they do not have the ability to manage so you end up with incompetent "yes" people and others that are promoted for cultural or nepotistic reasons. The company is seen as a laughing stock by other mid-size competitors and everyone with any talent or ability has either already left, was fired, or is actively looking for something else.
