Heads of HR departments consistently make sexist and ableist comments, harass employees, there are zero accessibility options for disabled employees, and zero tolerance for diversity, open-door-policy is just for show, managers tend to be unresponsive when approached from people with lower ranks, and since these problems start at the top, there is nobody to report these abuses to, so it's likely they'll never change.
