Incredibly poor management and managers who completely lack proper people and managerial skills, no structure, adequate training not provided, new staff thrown into their roles, software riddled with bugs, unhappy customers, bugs still in software "fixes," no diversity in the workplace, poor morale, no accountability for management or staff, zero appreciation for employees and the work that they do, poor communication across teams, double standards throughout organization, sell products to customers that don't yet exist that can't be delivered on, launch new features without fully testing use cases and delivering faulty products to customers.
