Leadership clique with an ego that's out of control, will throw team members under the bus in front of clients, and frequently belittle staff. Toxic office culture. No work/life balance. If you're over 35, forget it. If you don't get wasted at work functions, forget it. If you have kids, forget it. No processes. No time given to work - often loaded up beyond realistic workloads, even considering it's an industry that still wears the "I pulled an all-nighter" badge. If you're quiet and like to consider things instead of shooting from the hip don't join. If you challenge ideas, you're labelled as negative. If someone who's visa depends on their job to be able to stay in the country, leaves without having a job to go to, you know there are serious problems.

There's little or no processes and tools for running projects and as a result projects are usually a shambles which leads to severe scope creep and overruns, not to mention frantic producers. Despite this being a fairly regular occurrence, they don't seem to learn from their experiences. There's a lack of collaboration amongst teams and offices which is a huge shame as there's a lot of talented individuals that could add a hell of a lot of value to all sorts of projects going on around the world.
