Toxic & fake workplace. The main focus is catering to the supervisor's ego and bonding via gossip about WHO needs to improve instead of WHAT needs to improve. The Status Quo is fiercely protected. No matter how simple an improvement in a process would be, front-line workers are blamed instead.
Workers are faced with highly error-prone processes with no support from supervisors (nor from fellow coworkers who know to remain silent at all costs). Workers who mention anything wrong with the processes as opposed to placing blame get a target on their backs and are stonewalled and held in contempt by the supervisor (and her devotees).
Turning a blind eye and remaining silent -- regardless of how it might impact the patient -- is rewarded, while speaking up on behalf of patient care is aggressively punished. There is tremendous pressure to disregard policy and it is made very clear that embracing & encouraging below-minimum standards for patient care is the way to win favor here. Any sense of ethics, integrity, or high standards for patient care upsets the apple caret and will not be tolerated.
