April was National Minority Health Month - and what does that mean? It’s aiming for health equity through addressing the social drivers of health (SDOH) that represent 80-90% of the modifiable factors that impact health outcomes. Many of these drivers include community, social context, racism, and other factors that may impact minority populations more. While we can’t address all the systemic social and digital drivers of health, we can help each member, one by one, navigate their barriers to care. Our program is set up to: ⭐ Build rapport with culturally-sensitive and informed coaches ⭐ Engage with contextually-relevant program experience ⭐ Create commitment with relevant clinical guidelines It’s time to believe in better health.
