Systems and processes routinely rolled out without proper consultation with the staff who will be using them, resulting in issues for customers and staff alike. Lessons still not learned.
High staff turnover, largely due to the CEO micromanaging every aspect of the business, including all salaries and bonuses. While undoubtedly very good at his job, he consistently fails to take advantage of the talents of the great people that work for him. Unless your department head/director has some clout and can push back on the CEO to just allow you to get on with work, your department is likely to become quite dysfunctional.
I've seen some very deserving people go without bonuses despite putting in incredibly hard work, for what is quite frankly ridiculous reasons. This made a terrible and lasting impression on my entire team and was a severe blow to morale.
Also nepotism at play. They're not necessarily bad in their respective roles, but they're not necessarily great either.
