Where to start.. Pay was very poor, even for the care industry.

Illegal and unsafe health and safety practices such as wooden door stops instead of door guards (removed when inspections occured), No signage or lock on Coshh cupboard, decantering chemicals into unlabeled bottles.
Abusive practice (use of non prescribed restraint such as holding supine to the floor, despite staff being trained in studio 3).
Poor managers put in place as they would not challenge upper managements bad decisions. Good managers left because they weren't allowed to implement good practice because of the cost before care philosophy.
A multitude of medication errors that were covered up and never addressed by management.
Vehicles frequently breaking down due to improper servicing and very suspicious issuing of MOT certificates (service only ever used one mechanic that was about 15 miles away from the site).
Staff dismissed on the spot for minor misdemeanors. No following of fair procedure or aligning with acas code of conduct.
Treatment of staff ranging from poor to horrendous. Female staff members subjected to unwanted and inappropriate sexual contact from one particular manager. Eventually addressed but not before causing trauma to multiple different women.
Replacement culture. No interest from the service in retaining staff. Train them up, work then into the ground, watch them leave, fill the vacancy and repeat.
