Serious lack of clarity from HR on benefits package, PTO and work-remote policies that result in unfair (and non-negotiable) penalties. Highly inappropriate handling of sensitive matters (mental health, disability, layoffs, etc.) by HR / management that perpetuate a toxic culture and workplace gossip.
