Poor upper management, incompetant department supervisors, often supervisors are promoted from within the company after a certain number of years of employment (most without managing/supervisory background), concerns/complaints not kept confidential, not taken seriously or considered "personal attacks", poor raises (1-3% increase annually), high Patient Service Representative turnover, cameras installed without notifiying employees, once you are on the radar your days are numbered as a papertrail is started looking for a reason for termination
