Management across departments doesn't communicate properly with each other before pushing for changes, Some managers barely knew how to manage a team, had awful speech capacity and could not delegate (or adjust them) tasks among the team members, The company keeps creating costs that are completely unnecessary (e.g. new offices just for people to sit around), even though the business is struggling to break even.
