You have to apply for the commander pool and be accepted. Once in the pool you are eligible for a command position. Senior leaders will make a determination based on the pool who they recommend to commanders in the field to fill positions.

Question 1
no verbal questions...position was determined by performance report review
I applied online. The process took 4 months. I interviewed at US Air Force (Washington, DC) in June 2016
Interview
The "interview" process was more of a detailed package review. You submit a commander's application package which included items such as your letter of intent (i.e. cover letter), letters of recommendation, performance reports, picture, location preferences, resume, etc. Packages were reviewed by a headquarters level panel that first determined if your were selected to be a commander. If you were selected for command, you were subsequently matched to a location. Depending on the type of command position, there may be interview involved.

I applied through an employee referral. The process took 6 months. I interviewed at US Air Force (Oklahoma City, OK) in Jan 2014
Interview
Was called via Telephone from a 2-star General Officer and asked a few personal questions not appropriate for commercial asking (what do you think about coming to base X, how will your wife feel, do your children think they will like going to base x). It was followed by an in-person interview.
