Phone screen by a recruiter, call with the hiring manager, then onsite interview (panel). I enjoyed speaking with the hiring manager. By the time I had my onsite interview, I think they had decided to hire another candidate. No one on the panel showed up except the hiring manager. They either no-showed or cancelled at the last minute. I put in a LOT of work in on a test assignment (8+ hours) that I am pretty sure she didn't look at. Lesson learned. They left me with silence for a week after the onsite meeting then I got a one-line email from the recruiter with typos telling me I didn't get the job. They did not respect my time or make me feel valued as a candidate.
