Basics still not in place - so many basic things, like a progression framework, two years and it's still extremely basic, generic and not used. Financial forecasting is constantly wrong, and resourcing is chaos: people left on projects forever, or moved around with little notice.
Little recognition of good work - there's no way to recognise good work beyond a popularity contest shout out. No formal bonus system. People are able to coast, or even do bad work and let everyone else carry them.
