A Company That Lost Its Way

Pros

The mission is great but they lost how to execute it properly.

Cons

I worked at Options Medical Weight Loss, and while it once had potential, it has become entirely too sales-driven and mismanaged. The focus is no longer on patient care or employee well-being—it’s all about hitting unrealistic sales numbers, and if you don’t, you’re either pressured with scare tactics or cut loose. Medical exams? Barely 10 minutes long. This is terrible for patient care because it doesn’t allow enough time to properly assess a patient’s medical history, address their concerns, or create a truly personalized plan. Weight loss isn’t one-size-fits-all, but these rushed appointments make it feel that way. Patients deserve more than a quick in-and-out visit that prioritizes sales over real health outcomes. The food they push might help with weight loss, but it’s not necessarily the healthiest. And don’t expect stability—the company constantly rolls out changes without employee feedback, only to backtrack when their “brilliant” ideas fail and negatively impact both patients and staff. The work environment is incredibly clique-based, with clear favoritism at every level. If you’re not part of the inner circle, don’t expect to advance—no matter how hard you work. There’s also a noticeable lack of diversity, especially in corporate. Hiring practices make it clear that there is little to no effort to bring in people of color, and there’s an unspoken but obvious bias that creates an unwelcoming atmosphere for minorities. The commission structure is changed frequently, and never in a way that benefits employees. Clinic managers are forced to work overtime without additional pay, and corporate support is nearly nonexistent—you’re on your own when problems arise. They’ve slashed the marketing budget, pushing that responsibility onto already overworked managers. As a result, consultations have dropped significantly, but leadership still wonders why sales are down. Instead of fixing existing problems, they continue pouring money into opening new clinics while current ones struggle. The greed has become undeniable—mass layoffs, canceled holiday parties (“not in the budget”), and a complete disregard for employee morale. If you’re considering working here, be prepared for instability, favoritism, unrealistic expectations, and little to no support. They used to care about their employees, but now it’s all about money. Proceed with caution.

Going Downhill

Pros

Coworkers and patients are great.

Cons

Explanations on commission payout are impossible to get, so even if your check is wrong it will never be fixed. Rules are constantly changing and impossible to follow. HUGE pay cuts happened this year under the excuse that "this will promote you to go to more events to recruit more patients". My honest opinion is they are trying to save as much money as possible to open new clinics, while doing it at the expense of their own employees paychecks by giving them impossible to hit goals. The best employees in my same position will likely make 20-30k less per year on the new commission structure.

Pros

The work itself with patients is meaningful. You feel like you make a difference in the world.

Cons

It’s really disappointing to see how much this company has gone downhill. The work itself is meaningful, but leadership and corporate have made it hard to trust the organization. And if you don’t trust your employer, it’s nearly impossible to feel secure in your job or succeed in your role. Constant changes happen with little to no notice, affecting not just staff but patients as well. CST expects clinic staff to handle all the fallout from these changes, so corporate doesn’t have to deal with unhappy patients. Turnover is out of control. In my five years with the company, I saw four different COOs, leading to constant instability. Clinic associates are given an overwhelming amount of responsibility, and stress levels are always high. I know multiple people who had to go on anxiety medication because of this job. Having clinic managers work ALL clinic hours is crazy. Burnout is inevitable. At one point, Dr. Walker wanted this company to be the best place to work, and there was a real effort to take care of employees. That “people plan” disappeared within a year without any explanation. It used to feel like you were part of something, helping the company grow. But then, greed took over. The new COO made it clear that “we are creating a team, not a family.” Loyalty and hard work don’t matter anymore—only the money you bring in. Either you’re on CST’s good side, or you’re not. There’s no in-between. On top of that, the compensation and commission structures are always changing, making it impossible to feel financially secure. You can’t plan your future when your paycheck is unpredictable. Job security is also gone. Layoffs and leadership changes happen so often that no one feels safe. Even basic needs, like taking a lunch break, aren’t guaranteed. Without preplanned lunches, last-minute bookings often fill up the schedule, leaving no time to eat. There were many days I went without lunch at all. Leaving was a tough decision because I truly believe in the work being done for patients. But after I left, I realized how toxic the environment had become. Leadership keeps making empty promises, leaving you thinking things will get better, but they never do. They will say what they need to and lie to get what they want out of you. And when problems arise, instead of taking responsibility, corporate blames clinic staff. Nepotism is also a huge issue. The new leadership team is filled with people from the same previous company, and favoritism is the only way to move up. Meanwhile, corporate employees who have been there for 9+ years continue to get away with unethical behavior because of their connections. When negative employee reviews started rolling in, corporate refused to acknowledge the real issues. Instead, they blamed a single clinic, claiming the reviews were just from people upset about a manager being fired—completely ignoring the fact that employees across the company were unhappy. It’s truly sad to see what this place has become.
